Objectives
Contents The course is divided into three parts:
1) The first part outlines the general principles of reproductive and developmental toxicology and pharmacokinetic changes that drugs may undergo in pregnancy.
2) The second part describes methods of identifying teratogens, the major mechanisms underlying the teratogenic effects of drugs, and the risk associated with the use of drugs in pregnancy in relation to the Food and Drug Administration (FDA) classification
3) Part Three addresses the major issues and recommendations related to the use of over-the-counter (OTC) medications, herbal products, cigarette smoking, and substances of abuse in pregnancy and concludes with a summary of the principles underlying the prescription of medications in pregnancy in clinical practice.
Extended Syllabus Part I
General principles of reproductive and developmental toxicology: male and female reproductive toxicology, critical periods of susceptibility, dose-response relationship, and threshold concept. Pharmacokinetics in pregnancy and transplacental passage of drugs.
Part II
Types of teratogens, principles of teratogenesis, methods of identification of teratogens, major mechanisms of drug teratogenesis: antagonism of folic acid metabolism (competitive inhibitors of the enzyme dihydrofolate reductase, anti-epileptic drugs); destruction of neural crest cells (retinoid drugs); endocrine disruption (diethylstilbestrol); oxidative stress; vascular alteration; specific receptor- or enzyme-mediated teratogenesis (drugs acting on the renin-angiotensin system, statins, nonsteroidal anti-inflammatory drugs, anti-depressant drugs); FDA classification of risks due to the use of drugs in pregnancy

Over-the-counter (OTC) medications and pregnancy; medicinal plants in pregnancy; herbal products in pregnancy; cigarette smoking and substances of abuse in pregnancy; prescribing medications in childbearing and pregnancy: principles and clinical practice

Other important tools for the individual preparation are represented by the consultation of the "Medline" and some magazines of high scientific level and of pharmacological and pharmacological-clinical interest. Specific bibliographical indications will be provided during the course.
Teaching Methods The course includes lectures and interactive lessons.
The course aims to provide the theoretical basis necessary for a reasoned understanding of the mechanisms underlying the teratogenic effects of drugs. At the same time, particular care will be taken to link the theoretical knowledge of the risk associated with the use of drugs in pregnancy according to national and international guidelines with the principles of drug prescription in childbearing and pregnancy in clinical practice.
Evaluation methods Verification of learning:
The learning will be evaluated through a final examination (multiple choice quiz) aimed at providing a balanced assessment of the achievement of the objectives of the Core Curriculum). The modality of the exam and its evaluation are indicated by the teacher at the beginning of the lessons.
